In my article on this subject (http://www.content-complete.com/2007/01/29/ergonomics-for-the-content-worker/), I focus primarily on lessons learned from my own observations and experience. It's nice to see so much of that affirmed in the more...shall we say..."credentialed" sources you've cited here.

[...] Stop what you’re doing. Don’t move and just take a second to observe how your sitting in your chair. Comfortable? Probably not. How about your eyes? Been staring at this screen for a few hours now? Time to get up for a break. When you get back read this Devlounge post about a healthier workspace. [...]

I happened across the site http://www.desk-trainer.com when I was looking for some ergonomic and stretching information, and
when I tried it out, I was really impressed with the results. They have short exercises you can do at your desk to fight carpal tunnel, back pain, etc. They use a slick little flash animated guy to lead you through the exercises.

I had a sharp pain in my neck one day (which I usually never have), and I did a neck exercise for 5 minutes, and the pain was GONE and didn't come back. I was so impressed, I have been telling my friends about it. This has worked for me.

To add to your "refresh rate" section: LCD/TFT monitors are also a lot better for your eyes because they are progressive displays. This means they don't literally refresh but update pixel at a time and you don't get any flicker.

Getting the brightness of your screen correct also helps. Some people, me included *blushes*, tend to think "brightest is best" but too bright and you'll find yourself squinting a little.
